Initial Examinations are done on a first-come, first-served basis. No appointment is necessary.
This also applies to applicants referred back by the New Zealand Embassy for further tests.
Applicants who have been scheduled for additional examinations (e.g. Sputum collection) and/or DNA Testing must come on their designated examination date and time.

- Valid Passport
If not available, the following identification documents may be presented:- national ID (SSS, postal ID, voter's ID, PRC ID, driver's license) or
- school ID with birth certificate
- Two (2) PHOTOCOPIES of valid passport/ national ID/ school ID and birth certificate
- 6 Passport size photos (white or blue background)
- Letter/Medical instruction for Medical (if available)
Minors (15 years old and below) must be accompanied by their parent or by a legal guardian who has thorough knowledge of the child's medical history.
The parent or legal guardian must present a government issued ID (original and 2 photocopies)
In case the companion is not the legal guardian, the following documents must be presented:
- Letter of Authorization or Special power of attorney stating that the companion is authorized to sign on behalf of the parent
- 2 photocopies of the Parent's government issued ID
- Authorized companion's government issued ID, original and 2 photocopies
Whenever applicable, please bring the following:
- Old chest x-ray films not less than 3 months prior to the medical examination
- Medical certificate regarding a previously-treated or currently managed clinically significant illness The medical certificate should contain the following information:
- Medical Treatment
- name of medication (brand and/or generic name)
- dosage
- duration and dates of treatment
- surgical procedure done
- complete final diagnosis
- outcome of treatment or treatment plans and prognosis, if available

MEDICAL EXAMINATION FEES
ANNOUNCEMENT
INCREASE IN NEW ZEALAND MEDICAL FEES BEGINNING JULY 1, 2018
Beginning July 1, 2018, there will be an increase in the visa medical fees for New Zealand visa applicants. Please see table of fees below:

- Chest X-ray Only

0 to 4 years old | PhP 2,420.00 |
5 to 10 years old | PhP 2,650.00 |
11 to 14 years old | PhP 4,370.00 |
15 years old and up | PhP 13,350.00 |
TB screening and treatment are included in the Full Medical Examination, while all non-TB tests will be charged accordingly.
New Medical Examination Fee - PhP 2,530.00
Specialty consultations available within the clinic are performed free of charge. Referrals from other clinics will be charged accordingly.
St. Luke's Medical Center Extension Clinic accepts cash and credit card payments at the clinic cashier. Applicants who register online may also pay via Security Bank Bills Payment Facility.

REMINDERS:
- Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state. There are restaurants near St. Luke's Medical Center Extension Clinic and drinking fountains are available within the building.
- Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period. - Medical results and will be released according to the physical examination, x-ray and laboratory results, which will be conducted and processed in accordance with the guidelines set by Immigration New Zealand.
- The medical report and chest x-ray are submitted by St. Luke's Medical Center Extension Clinic released to the applicant, to be directly submitted to Immigration New Zealand.
